Dear Parent/Carer,

Year 9 Teenage Vaccinations

We recently sent your child a consent pack for their teenage vaccinations which are routinely given in school in year 9, please can you complete the consent form and return to school. If you have lost or misplaced the form, please contact the school immunisation team on 01274 221269 to give verbal consent or ask for another form.

Please note these are very important immunisations which are a continuation from your child’s baby immunisations and will provide them with lifelong protection against Diphtheria, Tetanus and Polio (one injection) and Meningococcal/Meningitis ACWY (one injection).
